Children with Special Needs Program

Child Find

The goal of Child Find is to ensure that all children in New York State have a physician. Qualified staff work with families, physicians and support agencies to offer developmental screenings & support to all children birth to age three who reside in Lewis County.

 

The Early Intervention Program (Birth-2),

The Early Intervention Program provides screenings and evaluations for children birth to age two to help identify developmental delays. Once your child is determined eligible for Early Intervention, some of the services availble include: physical therapy, occupational therapy, speech therapy, and special instruction;

 

 

The Preschool Special Education Program (3-5 Years),

This program focuses on the educational needs of children ages, 3 to 5, until they are Kindergarten eligible and is administered through the local school district in which the child resides. Children who are eligible for preschool services have a qualifying developmental or learning delay. Services are provided at no cost; and

 

 

Children with Special Care Needs (CSCN) Programs assist children from Birth to age 21 to reach toward and achieve developmental and educational goals and includes:

The Physically Handicapped Children’s Program(PHCP)

  • Diagnosis & Evaluation
  • Medical Services
  • Dental Services

PHCP was created to ensure access to health care through diagnostic evaluation and services for chronically ill and disable children.

    • Dental Rehabilitation Program

The Dental Program is designed to offer help to correct severe physically handicapping dental conditions, such as cleft lip and palate or severe malocclusions.

These programs are used for children when there is no other payer source for the medical services they need or when current insurance does not cover all the costs incurred.

    • Children with Special Healthcare Needs Information & Referral

We can also assist with accessing health and non-health care services. We can help with obtaining insurance, educational resources, social program assistance (public health services, social services, WIC, food stamps, SSI, transportation, respite services), and medical equipment.
